The $0 columns are the trap and the save. Tally Free and Google Forms are a save if the job is answers, not a branded funnel. Fillout Free is a save until response 1,001\. Typeform Basic at $28 is a trap the week a launch dumps 120 leads on a 100-response cap. Jotform’s $34 climbs with a second *login* or a BAA, not headcount.

## The HIPAA rule the intake form has to survive

Software does not make a patient form legal. A BAA does.

A vendor that creates, receives, maintains, or transmits PHI for a covered entity is a business associate. The covered entity may disclose PHI to that vendor only with a written BAA. A cloud form that stores a medical history is that vendor.

Keep PHI off Typeform Free, Tally, and Google Forms. Put the intake on Jotform Gold or Typeform Enterprise. A payment form is a PCI question — Stripe processes the card; the builder is not the merchant account.

**The billing unit is not the job.** Typeform meters responses and seats. Jotform meters forms, submissions, and (from Enterprise) users. Fillout meters responses with unlimited seats. Tally meters neither on Free. Google and Microsoft meter suite seats.

3. If a conversion page: Typeform Free to mock it. Pay Basic at $28 only for 100 responses. Pay Plus at $56 for branding and 1,000.
4. If templates, payments, and a BAA: Jotform Bronze at $34 without PHI. Gold at $99 with PHI. Do not put PHI on Starter.
5. If the team already lives in Gmail or Microsoft 365 and the form is internal: Google Forms or Microsoft Forms. Do not buy Typeform for the lunch RSVP.
6. Put one real week through the trial: one submit, one payment or signature if that is the job, one Sheet or Airtable row, one embed. If that fails on a cap, a missing BAA, or a card field that is not on Free, change lanes.
